Great Lakes Pain Center appreciates the referral of your patient with pain. We want you to know that we intend to work with you and your patient together in finding relief for their pain. We will do everything possible to make sure the lines of communication are open and clear regarding the treatment and progress of your patient

To make the referral process go as smoothly as possible we ask that all referrals include the following records:

1. Patient Demographic Sheet
2. Current medication list
3. Last 2 Office Visits
4. Recent Imaging of area of concern
5. Conservative care notes: Physical therapy notes, chiropractic notes or any other alternative therapy used to alleviate pain
